Innovation Award Program

Update on March 9, 2018: As of Monday, March 5, we have emailed a confirmation to all lead applicants that submitted an Innovation Award application confirming receipt of their submission. The email was sent by InnovationAward@onecityhealth.org. If a lead applicant submitted an application and did NOT receive a confirmation, please reach out to the OneCity Health support desk at ochsupportdesk@nychhc.org or 646-694-7090 right away.

Program Background

During the first half of the New York State DSRIP program, OneCity Health worked with partners to develop an Integrated Delivery System and implement targeted programs to meet DSRIP goals. As part of the evolution towards a value-based payment environment, OneCity Health launched the $5 million Innovation Award program on January 29, 2018. This program will support Performing Provider System (PPS) partners in designing and implementing innovative programs to improve the health of the patients and clients we serve and reduce avoidable use of the health care system.

And in the spirit of the DSRIP program – OneCity health strongly encourages collaboration among partners (click for partner contact list) and focusing on the social determinants of health. All partners with a signed Master Services Agreement (MSA) as of January 1, 2018 are eligible to submit an application.

OneCity Health expects to fund up to 10 awards in total; awards are expected to be in the range of $250,000 – $1,000,000 per selected application.

Submitting an application: Please submit all applications to InnovationAward@onecityhealth.org.

If an applicant is having issues completing the application in the PDF provided by OneCity Health, they can provide all information in a Word Document that adheres to the requirements and page limits outlined in the application. If possible, please convert the word document to a PDF prior to submission.
