NYS DOH Survey on the use of Social Determinants of Health Assessment Tools

The New York State Department of Health (NYS DOH) has released a 10-question survey on the creation and use of Social Determinants of Health (SDH) assessment tools. The purpose of this survey is to get a sense of the variations of SDH assessment tools currently being used across the State.

The NYS DOH has asked for interested organizations to complete this brief survey by September 27th.

A SDH assessment tool is defined as a survey or questionnaire that is administered to patients/clients/members to assess their level of vulnerability or accessibility to needed social determinants of health interventions including but not limited to: food, housing, medication, child care, legal help, domestic violence services, employment, smoking cessation, sobriety services, and education.
