To establish a robust integrated delivery system, OneCity Health’s Performing Provider System (PPS) must build and maintain a robust partner network that offers a comprehensive continuum of services throughout New York City. Key elements of this effort include assessing and supporting the financial health of our partner network.
In accordance with guidance from the New York State Department of Health, OneCity Health must develop and submit a sustainability strategy by March 31, 2016 to explain how we will support our partner network throughout the Delivery System Reform Incentive Payment (DSRIP) program.
OneCity Health’s Business Operations and Information Technology Governance sub-committee has approved the utilization of a brief survey to obtain a baseline assessment of the financial health of our partners. The survey consists of ‘yes’ or ‘no’ questions and should take less than 10 minutes to complete. All survey responses will be confidential, and data will be aggregated and de-identified in reporting. The survey was released on Monday, February 22, 2016 via email to each partner’s designated Master Services Agreement (MSA) signatory. They will submit the survey electronically on behalf of your ogranization. Here is the PDF version of the survey.
