Project Participation Opportunity: Hospital – Community Bridge Partner

Achieving the goals and objectives of an integrated delivery system requires greater communication and collaboration between the health care and social service sectors, including increased and enduring hospital-community partnerships.

Through a Project Participation Opportunity (PPO), we are seeking to contract with a “Hospital – Community Bridge Partner” that will help facilitate relationships between participating hospitals and community-based organizations (CBOs) in the OneCity Health Performing Provider System (PPS).

The selected partner will work closely with OneCity Health to develop and cultivate relationships between participating hospitals and CBOs in the same geographical regions, focused on increased collaboration, innovation and performance improvement on DSRIP Outcome Measures.

The PPO Interest Form outlines the opportunity, including background, criteria and next steps to express interest. Partners interested in applying should submit a 2-page description of their experience facilitating relationships between health care providers and social service providers and how they will leverage such experience to support this initiative. Applications must be submitted by 6 p.m. on Friday, June 22, 2018 to the OneCity Health Support Desk at with the subject line “Hospital – Community Bridge Partner Application”.

This initiative will begin upon execution of a Schedule B and will end on March 31, 2020.
