November 2017 Phase II OneCity Health Partner Reporting Manual Now Available

To assist our partners with completing all remaining metrics that are included in their Comprehensive Schedules B for Phase II (which runs through December 31, 2017), we have developed the latest version of the Phase II OneCity Health Partner Reporting Manual.

The November 2017 Phase II OneCity Health Partner Reporting Manual replaces the version released in September 2017 and reflects the following updates:

  • Additional guidance for three Phase II Comprehensive Schedule B Process Metrics due through the end of Phase II, including:
    • PM023 (Integrated Delivery System) – Page 44
    • PM025 (Integrated Delivery System) – Page 50
    • PM027 (ED Care Triage for At-Risk Populations) – Page 52
  • New guidance on reporting on a Distinct Schedule B Metric DSB_PCMHFee (Reimbursement of Patient-Centered Medical Home (PCMH) Application Related Fees) – Page 64

  • Additional guidance for seven Phase II Comprehensive Schedule B Patient Engagement Metrics
  • Updated guidance on creating accounts, navigating and resubmitting metrics on the OneCity Health Partner Portal

  • June 30, 2017: Complete metric PM007 (Phase II Partner Network & Gap Assessment Survey). It is now available to complete in the OneCity Health Partner Portal
  • July 11, 2017: Attend our next monthly OneCity Health Partner Webinar. The webinar will be held from 2 – 3:30 p.m. Details are on our website Events Calendar
  • August 15, 2017: New deadline for both the NPI Survey and Phase III Contracting Survey

Updates from the PAC: Phase I Metric Submissions

At our March 8, 2017 Project Advisory Committee (PAC) meeting, our partners offered feedback and follow-up questions on a number of resources and ongoing initiatives that we have introduced to further develop our integrated delivery system. Specifically, our partners suggested:

Below please find our final update, which is a recap of our network’s metric submission performance in Phase I. We will also continue to provide updates on these topics on our monthly OneCity Health Partner Webinars, which are also a forum for additional Q&A with our partners.

Phase I Submissions for Comprehensive Schedule B Metrics
Throughout Phase I (July 1, 2016 – March 31, 2017), we asked partners to submit a variety of Operations & Outcomes, Participation, and Data & Tracking metrics that were included in their Comprehensive Schedule B, and were collectively referred to as Performance Metrics. All metrics were detailed in the OneCity Health Partner Reporting Manual and often reviewed during monthly OneCity Health Partner Webinars.

Across all Comprehensive Schedule B projects, the overall submission rate for closed Phase I metrics was 55 percent, as of April 30, 2017. For approved metrics, the invoicing rate was 80 percent, as of May 31, 2017. Throughout and beyond Phase I, OneCity Health conducted outreach and provided technical and programmatic support to our partners to enable their metric submission and/or remediation.

Thank you to those partners who submitted their metrics on time and with complete and accurate information. Phase I was our first step working with our partners to report on metrics – and a learning opportunity for all of us. For instance, we needed to develop and orient partners to the newly established OneCity Health Partner Portal and support desk.

These areas for improvement notwithstanding, we appreciate the diligent efforts of our partners to date.

Many of the Phase II metrics (which we are now collectively referring to as ‘process metrics’) are incremental steps in driving transformation activities, which drive our collective success in achieving positive health outcomes for our patients, members or clients. Thus in Phase II, it is imperative that our partners intensify their efforts to consistently submit their Phase II Comprehensive Schedule B metrics on time and with complete and accurate information. As we described in a previous update, OneCity Health has implemented significant improvements to the OneCity Health Partner Portal, making it easier to navigate, identify metric due dates, and submit an invoice, since it is now auto-created upon metric submission.

We will continue to share updates on meeting contract metrics during our monthly OneCity Health Partner Webinars. The next one is scheduled for June 13, 2017.

The Reporting Manual provides information about reporting on all Phase II project metrics and is organized into two major sections:

  • Metrics
  • OneCity Health Partner Portal Overview

In addition, page four of the manual includes a glossary of key terms, and page five updates partners on the new process to invoice. In our April 11, 2017 OneCity Health Partner Webinar we discussed invoicing and the redesigned OneCity Health Partner Portal.

  • As soon as possible: If our partners have not yet completed DSRIP compliance training, please do so immediately. Background on DSRIP compliance, instructions on how to easily meet this requirement and the attestation form to submit are available here
  • By April 20, 2017 (but as soon as possible): Our partners need to submit their Workforce Impact Survey to the OneCity Health support desk (email it to ochsupportdesk@nychhc.org) and complete the attestation on the Portal. However, we ask that our partners submit the completed Survey as soon as possible
  • April 26, 2017: Five Phase I Patient Engagement (PE) metrics are due to OneCity Health
  • April 30, 2017: Four Phase I recurring Operations & Outcomes (OO) metrics are due to OneCity Health
  • May 9, 2017: Attend our next monthly OneCity Health Partner Webinar. The webinar will be held from 2 – 3:30 p.m. Details are on our Events Calendar
  • By May 15, 2017: Partners need to sign their Comprehensive Schedule B and complete the attestation on the Portal, otherwise they will forfeit participation in Phase II contracting
  • If our partners have questions about completing metrics or submitting reports, please contact our support desk

New Guides and Resources to Assist Partners

To continue to assist our partners with reporting and other requirements, we’ve added a number of technical and informative resources to our OneCity Health Partner Reporting Manual page.

As we move into Phase II contracting, we’ve added instructions to help sign OneCity Health contracts, and basic tips to help with reporting and using the OneCity Health Partner Portal. We’ve also included a glossary of general Delivery System Reform Incentive Payment (DSRIP) terms.
