Phase III OneCity Health Partner Webinar Dates

With Phase III contracting (January 1, 2018) on the horizon, we have scheduled the next slate of dates in our OneCity Health Partner Webinar series.

As we’ve done through Phases I and II, we will continue to use these webinars to discuss contracting and our partners’ Comprehensive Schedules B, the latest resources and practical steps for implementation, and how our partners can meet their reporting requirements. In Phase III, as we continue our shift toward improving health outcomes, we will also focus on topics that strengthen our integrated delivery system and tie our partners together, such as value-based payments, identifying community resources for patients through NowPow and quality improvement initiatives.

We will continue to hold OneCity Health Partner Webinars on the second Tuesday of every month. Here is the full schedule:

Webinar Available to View: Collaborative Care for Depression

America’s Essential Hospitals recently hosted this webinar on collaborative care for depression in the primary care setting.

Led by Jessica Black, MPH, MSW, Program Manager, Collaborative Care and Kathleen Tatem, MPH, Data Analyst, both of OneCity Health, attendees learned how NYC Health + Hospitals developed collaborative care tools to drive operational workflows and quality improvement across multiple sites. As a result, the depression improvement rate at NYC Health + Hospitals increased from 17.7 percent to 58.2 percent.

This past June, NYC Health + Hospitals was awarded the 2017 Gage Award for Innovation and Excellence from America’s Essential Hospitals for implementing collaborative care for depression in the primary care setting. Read more about the award and program here.

“Value Based Payment: The Basics” Training Now Available

Our initial value-based payment (VBP) training, “Value Based Payment: The Basics”, is now available to view and download. This training reviews key concepts and explains how VBP contracting can lead to improved care and lower costs. It also goes into more detail about OneCity Health’s upcoming support related to VBP.

Viewing or downloading this presentation will help OneCity Health partners complete Phase II Comprehensive Schedule B Process Metric PM017. Steps to complete the metric are detailed below.

There are TWO steps for OneCity Health partners to complete PM017.
  • First, OneCity Health partners are required to distribute this training in one of these forms (the recorded webinar to view OR the downloaded presentation) to their staff
  • They must then attest on the OneCity Health Partner Portal to having distributed the training materials to their staff. This counts as “attending” a VBP training for PM017

The deadline to BOTH distribute and attest is December 8, 2017.

Please note, VBP trainings offered by entities other than OneCity Health will not count towards completion of this metric.

  • Due December 8, 2017: Ten Process Metrics are due to OneCity Health. As a reminder, OneCity Health partners are only responsible for the metrics in their Phase II Comprehensive Schedule B. They should also be sure to attest on the OneCity Health Partner Portal for completing the VBP training (all partners) and NowPow training (some partners)
  • December 12, 2017: Participate in our next OneCity Health Partner Webinar – our last of Phase II. Phase III webinar dates will be posted to our website soon
  • Due December 15, 2017: For our participating CBO partners, the deadline to complete the organizational self-assessment

Partnership in Care Asthma Webinar Now Available to View

In this presentation, we discussed how a partnership between the primary care team and community health worker is essential for effective asthma management for children with asthma. Moreover, we highlighted how asthma self-management education improves patient outcomes, as community health workers utilize a written Asthma Action Plan tailored to the needs and literacy levels of the patient to reinforce patient education provided by the clinical team.

Throughout August, OneCity Health hosted a series of webinars on the Co-Location of Primary Care and Behavioral Health Services for our ten pilot sites currently planning for co-location. Screening Tools for Co-location of Primary Care and Behavioral Health Services
